National Addictions Awareness Week Volleyball Tourney

$200Siksika Mental Health and Community Wellness are excited to host our annual National Addictions Awareness Week volleyball tournament on November 26 & 27, 2021 at the Siksika Deerfoot Sportsplex.
Format: Co-ed – 4 men, 2 women per team (to a maximum of 8 teams)
Entry fee: $200 / team (must pay in full by November 25th). All proceeds will go towards the Siksika Shelter
Prizes, merchandise, player awards will be given to top teams and players.
Please note that all players and attendees who are eligible to receive the COVID-19 vaccine (born in 2009 or earlier) must be fully vaccinated (“fully vaccinated” defined as 14+ days after second dose of approved COVID-19 vaccine). Proof of vaccination records (QR codes) will be requested at the time of entry.
To get an Alberta verified COVID-19 vaccine record with QR code:
- Visit https://covidrecords.alberta.ca/home
- Complete the form – enter your Personal Health Number (Alberta Healthcare Number), Date of Birth, and Date (month/year) of COVID vaccination
- Submit the completed form
- Print or save the QR code for future use
It is recommended that Siksikawa save a copy of their “Proof of Vaccination” QR code to their mobile device, however if this is not possible, hardcopies can be printed at the immunization clinic located at the Deerfoot Sportsplex, Monday & Friday between 10am – 4pm, and Tuesday-Thursday between 10am-6pm.
